Adoption Finalization

This service is available to any child or youth in the legal custody of a county’s children and youth agency (CYS) who has a goal of adoption. Finalization services begin after the placement of a child, sibling group or an older youth into a pre-adoptive home. Payment for this unit covers the development of an Individual Permanency Service Plan, supervision of the placement by the affiliate, as well as a minimum of three supervisory reports. This applies to children or youth who are in either a foster-to-adopt or adoption situation; both statuses of families benefit from adoption-specific support and supervision. Supportive services, reports and home visits, as well as work with attorneys and courts, are included in the Finalization unit. An affiliate agency can invoice for this service once the adoption is finalized (decree is filed).

Adoption Finalization Extension

Effective July 2018, Adoption Finalization became an eight-month unit of service with the option for counties to refer for an additional four months at half the cost. The Adoption Finalization Extension continues all services provided to the child, the pre-adoptive family and the county agency with custody of the child prior to the issuance of a decree of adoption by the court. It may include, but is not limited to, supportive services, home visits, supervisory reports, and the work done with attorneys and courts to facilitate the adoption in a timely manner.
